Special Services: •EMG's: EMG'S are done in the comfort of our office. •Bone Density: Ultrasound of the wrist. •Women's Health: Annual pap and breast exam. •Pediatrics: Well Child Exams •Laser Surgery: For removal of moles and skin tags. •Weight Loss •Prescription Refills: We need at least 24 hours notice for any refill requests. •Referrals: We require at least 2 days advance notice for regular referrals. Be aware some MRI's and Cat scans may take up to a week to get a referral approved. •Results: Lab, X-ray and test results •Sick Notes: Out of work or school sick notes •Forms: Family Medical leave papers, out of work forms, Motor vehicle forms •Minor children: Consent to care of minors (anyone under 18 years of age) |
