Sick notes WILL NOT be given out to patient's unless they are seen by a provider or unless there are extenuating circumstances. 

• You will need to provide us with the dates of your absence as well as your expected return to work date. 
• If you ask the office staff to fax a sick note for you, it is still YOUR RESPONSIBILITY to make sure that your employer received the sick note. 
• It is also your responsibility to make sure that you come back for another visit ON OR BEFORE the return date on our sick note if you are to continue to stay out of work or school longer.
