Minor Children

If is required by the State of Delaware that minors requiring medical treatment have a relative caregiver (parent, grandparent, Aunt, Uncle), legal guardian or temporary custodian (with legal power of attorney with power to consent to medical treatment) to authorize the consent of treatment to ALL MINOR children. 

  • Our office must have a note signed by the parent (signatures will be compared to what's on file) or a phone number to reach the parent/legal guardian, unless the parent or legal guardian cannot be reached and the physician determines that if the minor is left untreated, there could be a threat to the minor's health or life.
  • Parents who routinely have friends or other family members bring in their minor child need to fill out a consent to treat minor child form.
